ILLNESS DECLARATION

I had read, understand, and agree that the child under my supervision is FREE from all illnesses stated below on every admission. Otherwise, Seven Evernew PLT has the right to deny my admission.

▪a temperature higher than 37.5°C or more
▪yellow discharge or crustiness around the eyes
▪inflamed puffy eyes, or red eyes (pink eye)
▪paleness, flushed face, or constant crying
▪lethargy
▪chicken pox
▪vomiting (two or more episodes within 24 hours)
▪diarrhea (within 24 hours)
▪a rash
▪severe sore throat
▪swollen glands
▪stomach aches (severe)
▪ear ache (severe)
▪headache (severe)
▪respiratory problems
▪head lice
▪discharge from the nose (excessive and accompanied by other symptoms)
▪discharge (eyes, ears, vagina, penis)

GENERAL TERMS

1.

I am 18 years old and above and I am eligible to supervise and held full responsibility for the child that I brought into the premise.

2.

I understand that Seven Evernew PLT has the right to request me to produce proof of identification (MyKad/MyKid/Passport) if deemed necessary.

3.

I indemnify Seven Evernew PLT against any claim relating to the loss of personal belongings or damage or personal injury to my child arising from the visit to TiniTin Town.

4.

I shall assume full responsibility and liability for any loss or injury that occurred during my visit.

5.

I understand that all notices, rules and regulations, and wall signs posted in TiniTin Town must be observed and followed.

6.

I agree and understand Personal Data Protection Act (PDPA) as shown under the privacy policy and agree to Seven Evernew PLT processing my personal data for the purposes and in the manners set out in the PDPA.

7.

I understand that Seven Evernew PLT reserves the right to alter any terms and conditions without prior notice.
